As of November 1, 2011, an updated ABN (Advanced Beneficiary Notice of Non-Coverage) is required to be implemented in replacement of the current ABN Form used. Only minor edits have been made to the new ABN for purposes of clarity.
Any ABN Form used on or after the implementation date listed above that is not the current version, will be considered invalid.
You can obtain the new ABN form here, as well as instructions for use.
It is recommended that offices review the current ABN manual to insure proper use in practices.
